You want the ThinkPad Integrated Bluetooth IV with 56K Modem BMDC-3 - FRU # 39T0022.
You will also need an antenna - FRU # 13N5179

Note: Check the BMDC slot in your machine for the presence of an unused bluetooth antenna. If it is there you do not need to install a new antenna. Be aware that the unused antenna wire may be underneath the current modem card, which you will need to remove to see it. Use the R52 maintenance manual for removals;

The card works on an XP stack. I already explained how to look for the antenna. It's a gray wire with a connector on the end of it. It goes from the BMDC slot to a spot behind the LCD bezel.

Oh sorry about that, I did not see any antenna and I even removed the card. So to install an antenna I have to take the plastic off of the the LCD Bezel and route the wire?
