Quick question...does this battery stick out the rear of the laptop or is it flush with the back?

It sticks out. Only the 4 cell is flush. The six and seven cell versions both are the exact same dimensions, as far as I can tell, but the 7 cell weighs more. They both stick out about 1/2" or so.

Furthermore, for me, this is the biggest design failure of the Z60t, Z61t, T61 14" WS and R61 14" WS models (maybe more). The 4 cell batteries are really only good for 2 hours when they are new, then they quickly deteriorate. By comparison, the T4x and T60 line had flush 6 cell batteries and the R40 had a flush 8 cell battery. That's what I'm talking about! These are laptops, not desktops or even desktop replacement laptops.
