This modem error, as it applies to Windows 2000 Pro, is discussed in KB 810979:

The same error can occur in XP sp2, but the fix is different, due to unavailability of TCP/IP "uninstall" in XP sp2. Justin T. Ho breaks it down nicely (for VPN, but still works with simple dial-up):
