What is the best:

Intel choices include a 3160, and a 7265. ???

Not sure about 3x3, but I'm using an Azurewave AW-CE123H. I don't have an AC network to test with, but it seems quite nice otherwise and includes BT 4. I did mainly purchase it for OS X compatibility, so I'm a little biased away from Intel chips.
Good luck!

Did you disconnect the Lenovo BT module?

How do you control it, ie., turn wifi & BT on/off?

Yes, I replaced the regular BT module with a webcam :)

The WiFi switch works fine, I'm not 100% sure if it also switches off BT as well though. I'd have to check.

Did you place this combo module in WWAN slot??? Common thing is that BT in combo modules use USB interface in miniPCIe slot and WLAN slot haven't USB lane.
Did you tested this module in WLAN slot? Did it worked?

I have the card installed in the right-hand slot (WWAN?). BT works fine there, but does not turn off with the HW switch.

Fn+F5 does not work either.
