Battery Gauge

In Windows Vista is there a way to get the battery gauge on the toolbar?

No. Your option is taskbar and/or tray and/or a widget/gadget (whatever the heck Microsoft calls those things).

What do you mean by "Vista toolbar"? The Sidebar? You can have the gauge there. The gadget is installed together with the Power Manager, you just have to open the sidebar, click on the "+" sign on top of it and drag the gauge gadget to the desired location on the sidebar.

On my t42 in the Start Menu toolbar there was a battery gauge near the system icons, but it was a toolbar. You know, like the Windows Media & Itunes toolbars.

Now it's appeared again? It's the "Power Manager Gauge." Is it only there when the power is low?

That's your taskbar. As far as I know it either setup to be there or to not be there. I don't think there is an in between setting that has it show up only on low battery.

Yes, the taskbar gauge can only be shown/hidden. The relevant option is in the Power Manager. Open it, make sure it is using the advanced/enhanced view (use the button in the top right) and go to the Options tab. The gauge is enabled by the first option there.
