I have one of these (4170 model with NVIDIA graphics). I'm embarrassed to admit it, but I'd never heard of expresscard until today. Am I right to think that my SD card reader is just an "expresscard" that can be replaced with something else, like a firewire port or an SSD?!

No, your SD card reader is just that: a slot in which to insert SD cards.
And Express cards are to be inserted in an Expresscard slot, which machines like yours could have instead of the card reader.
Whether they are exchangeable, I don't know (doubt it).

T420s' card reader is removable (just push and extract), the expresscard slot is 34mm.

ExpressCard is the replacement for the old PCMCIA and CardBus notebook expansion card interface. Various things can be attached to the system through ExpressCard expansion, such as USB ports, FireWire ports, Ethernet/mobile broadband cards, SATA ports, TV tuner cards, etc.
