1. What OS are you using? Windows? MacOS? etc.

2. Upload the picture on an image hosting site (flickr or similar) then post the link here or use [img] to embed it in your comment. This site is strict about image sizes, so if it's larger than 1024x768, post it as a link, otherwise you'll receive a warning from the moderators.

It might be an actual hardware issue. Remember that T420s is a decade old by now, and any form of malfunction is to be expected.
Can you show more details on that unknown device? Right-click on it and select "Properties". Post screenshots of the contents of each tab.

Thinkpads have a number of integrated devices connected via USB. From memory WWAN card, Bluetooth adapter and Camera / Microphone at the very least. Any of them can be failing and reporting as a mysterious device. Try enabling / testing such features to see if they still work properly.

Also, if the machine has been used in a hot and wet environment, chances are that the connectors have been corroded. I recently purchased some old stock ultra slim ITX PC cases off eBay and the machine I built using one was throwing weird errors in Windows. The problem was corrosion in the front panel ports (audio and USB). I took the whole module off and the problem disappeared. Good thing it's a normal PCB, so I just need to go and find new USB ports (I'll get good quality this time... the old ones probably were bottom of the barrel stuff with no gold plating) and re-solder them. You can't do that to a laptop motherboard (well, you can, but it's far more difficult) so I'd try to rinse it with distilled water if you find some sulfate.

I was also getting a lot of trouble with early USB 3.0 external controller integrated into the motherboard (DH61AG). Renesas I think it was. It kept interfering with the normal USB 2.0 ports. I tried a different driver version (newer, I do remember reading they fixed some incompatibility) and the system went back to normal. The T420s uses an external USB 3.0 controller as well, since intel didn't integrate one into the chipset until Ivy Bridge.
