I bought myself a simple laptop stand last week and it's been worthwhile in terms of keeping my eyes at a good level. Besides the external keyboard/mouse, that's pretty much all I do at my desk. However, laying on my stomach with the T40 on front of me in front of the TV has left me with slight neck strains a couple of times

Most of the time I'm either laying in bed (or on the couch) or reclined in my deskchair or my recliner when using my ThinkPad and I don't have many issues. When using my newer laptop which has a 15" wide aspect screen, I don't have the bed option as it's too wide to be comfortable laying down.
My favorite position is actually to lay stomach down on my floor with the laptop in front of me. Hardwood flooring is alot more comfortable than it looks.

As are tiles, even in winter when my mother would say they're too cold for her bare feet. Perhaps that's why I'm going to a chiropractor twice a week. Actually, since I've been hacking away at one keyboard or another for the past 30 years, the last two almost exclusively on laptops both at work and at home, the curvature in my neck is gone, the neck and shoulder muscles are tense, but thankfully, no CTS in my wrists. Laptops ARE not very healthy. The best thing you can do is take frequent breaks, practice some neck stretching exercises (ear-to-shoulder bends) and try to get your chair up high enough to keep your forearms parallel with the floor when typing.

Have you tried a neck roll pillow behind your head in that recliner?? They support the neck curve just right and prevent strain, it is amazing how heavy the human head is and the neck isn't able to hold that weight at an angle for long before straining.
