Bluetooth disappeared, unknown USB device on T450

Well, I just barely managed to solve a bizarre Bluetooth issue just now, and I'm posting it here for both my future reference and perhaps to help others.


I recently received a new T450 (to replace my 10yo Z60M, ha!) and everything's been going pretty well, notwithstanding some growing pains getting used to windows 8.1.

While trying to be productive today, my Bluetooth adapter disappeared. I like to use an external mouse and have a simple Logitech Bluetooth mouse. It had been working fine for hours, and then all of a sudden stopped, and all traces of Bluetooth on my laptop disappeared.

Checking device manager, I found that there was an Unknown USB Device (Device descriptor Request failed ) listed in the "Universal Serial Bus controllers" section, and that there was no Bluetooth section. That's odd, methinks. Googling around, I found that windows 8 attempts to provide more information about USB issues, and opening up the driver details revealed I had an error 43. The long story short is that their suggestion for fixing is to plug/unplug or otherwise remove power from the device and try again. I had not recently changed any drivers or software, or plugged/unplugged any USB devices, so I couldn't see how I could have corrupted a driver or anything else bizarre, so I tried a reboot with no effect, then a power down and cold boot with no resolution. I was going to pull the (removable) battery until I remembered that there's a permanent battery. So then I pulled up the HMM and was ready to tear the PC apart. Because I really didn't want to do that, I went back in to device manager, and all the USB root hubs caught my attention. I started disabling those USB hub drivers and re-enabling one at a time, and found that disabling the "USB Root Hub (xHCI)" caused the faulty USB driver to disappear, and upon enabling the USB Root Hub (xHCI), all of my missing Bluetooth device drivers reappeared and were functioning! Hooray!

Some glitch must have caused a disconnect of the Bluetooth adapter (maybe a power surge), and it failed to reconnect. Resetting the connection by disabling and re-enabling the hub above it fixed it. Strange that a reboot wouldn't.

It's great when such stories end well, and thank you for sharing it. :)

Hey Folks,

apologies for digging up this oldie. :?

It looks like I've ran into the exact same problem - Bluetooth mouse stopped working, Bluetooth toggle gone, no trace of Bluetooth in Device Manager and the above mentioned USB shenanigans.
Only differences, I'm working on a T450s and the solution above does not fix things.

Disabling "USB Root Hub (xHCI)" did in fact cause the faulty USB driver to disappear, but upon reenabling, the only thing reappearing was said USB driver. Still not a trace of Bluetooth. T

Tried rebooting and only then reenabling, to no effect.

Does anyone have any smart ideas? I've still got 1 1/2 years of warranty on the machine, but I'd rather just get it done and resume working with my mouse. :mrgreen:
