First of all, I hope this hasn't been discussed here before (yeah, I've searched).
I have an external USB card reader (SanDisk Extreme USB 2.0 Reader), which - at least it seems so - get correctly installed after plugging in. It can read both CF and MMS cards without a problem but it won't identify/read any SD card. Sometimes it doesn't get detected at all, sometimes asks for a format (the card is formatted - even two format methods were tried) but when I try to format it, I get an error saying that "Windows was unable to complete the format". Those are "standard", non-SDHD cards, one of them for ex. 4GB MiniSD in a mini2standard adapter, other an old 16MB Toshiba.
On the other hand, my PCMCIA card reader reads it ok - but it's sloooow, so I really need to get USB reader working for the 4GB card.
The reader worked fine even with SD cards with the good ol' winXP.
SanDisk doesn't supply any drivers, stating that the card should get installed automatically in Windows.
I was able to find many complaints about this in the internet but the only possible solution was this:
http://support.microsoft.com/?kbid=936825
...but I only get a notice "The update does not apply to your system".
Thanks a bunch for all the clues...
(using the T400 noted in my signature, with WinVista Ultimate, all the Lenovo and Microsoft updates installed)

Re: USB card reader won't read SD card
Those symptoms point to a defective reader. Had a similar problem with an Express slot reader that went bad. No card found, or prompted for a format to be done. The format could never be completed either.
Cell phones are my specialty. Got questions? Ask away.

Re: USB card reader won't read SD card
Yeah, I've got the unit replaced. Unfortunatelly, with a reader without a CF slot
