Why was there no X220s or 'proper' X230s? Did they ever exist?

The efficiency of two screens equally sized with equal numbers if pixels are equal. The time spent by a 4:3 user complaining about 16:9 is proportional to the inefficiency working with a 16:9 display, therefore the amount of useful work extracted is equal.

No, AFAIK. The closest thing to an X220s is the regular X220 chassis with a ULV processor and the single heatpipe heatsink (04W0436) that weighs only 34 grams IIRC.

As to "why": my guess is that the battery life was the biggest thing. The second Core generation (Sandy Bridge) that X220 is based on brought much improved power efficiency, which allowed the mainstream full-voltage CPUs to achieve comparable battery life to low-voltage CPUs of previous generations. Thus it was deemed unnecessary to have a whole parallel product just for the ULV CPU, although as axur-delmeria pointed out: customized-to-order X220 units with ULV CPUs and a lighter heatsink were, in fact, made.

There is an X230s with ULV CPU (limited to CN) similar to X240s.
