FRU 42W2028/42X4685 is the heat-sink you should be looking for, do note that it does not matter whether if it came from a 15.4" widescreen or a 14.1" T61. They are interchangeable without any modding required to get them installed.

These mentioned heatsink can be further modded too: take a T500/W500 fan with cover from the heatsink and put it on the T61 heatsink, this will increase the cooling efficiency. Best part about this is there is nothing needed about modifying the fan to fit the T61 heatsink, just install it as is. T500/W500 heatsink themselves are not compatible.

If you were referring to the 14.1" widescreen variant, no. The 14.1" widescreen heat-sinks are not interchangeable with the 14.1" standard heat-sinks, only the 15.4" widescreen's heat-sinks are interchangeable with the 14.1" standard heat-sinks.

It will only improve the 'efficiency' overall, by reducing the noise generated. I believe it produces less noise than a equivalent 15.4" widescreen/14.1" T61 heat-sink fan.
The slightly faster maximum fan speed (~5100 RPM) of the W500/T500's fan is also maybe a little helpful, that is if you do ever operate it at its maximum speed. Whilst the T61's fan tends to max around ~4500 RPM, but it is noisy for some people that are sensitive to noise. As far as I know, there is no need to swap the fan for a W500/T500's unless you are very sensitive or annoyed by the noise produced by the fan.
