I have a Lenovo ThinkPad T61 laptop, 6457-4UU and am having some difficulty with being able to use only one working USB ports. I seldom use the CD/DVD drive and would like to exchange that drive for a device that fits into the CD/DVD bay and gives me additional USB 2.0 ports. I don’t want to use an external USB hub. Recently I added a USB Express card using an adapter into the card slot, which gave me two more for a total of three working USB 2.0 ports; however the adapter sticks out, meaning it doesn’t sit flush with the laptop, but it does work. I tried using another Express card that WOULD seat flush with it, but it could not be recognized by Windows. This second card had USB 3.0 ports instead of 2.0 ports.....could this be the reason it didn’t work? Oddly enough, it did work on a ThinkPad T500. The T61 and T500 are practically identical and I purchased the T500 to replace a non-working T61, intending to simply swap the hard drive from the T61 and seat it in the T500 but it popped up the BOSD Upon booting. I then tried it on another T61 and it booted with no problem. What can possibly be the difference in the T500 that prevents booting from the T61 hard drive?
Dell has such a device that can be swapped with the optical drive and give me more additional USB ports, but as of yet, I’ve found no such device for the T61. Not even sure what the device is called...expansion bay for USB? Does it even exist?

I don't think there are any for T61.
Your express card should work just fine - I have such card and it works perfectly fine.

Check the insides of all USB ports.
Is the plastic strip inside missing?
Are any contacts visible?
Do they make contact with each other and/or with the USB casing?
One failure like that may disable all other USB ports!
Remove all power before you try to straighten anything.

The chipset driver and the SATA controller driver. I am sure that is very likely to be the reason, since the T500 and the T61 do not utilize the same chipset.
The T500 uses the PM45 or the GM45 northbridge that is paired with the ICH9M-E southbridge, but the T61 uses the PM965 or the GM965 northbridge that is paired with the ICH8M-E southbridge. If you were to think about it, the major difference in hardware alone is likely to cause a blue screen of death.
Unfortunately, I don't think such an Ultrabay Slim device exists for the T61. The closest that I can think of is the Ultrabay Slim serial and parallel port adapter.

USB2 twin adapters for the PC card slot of the 'inside' variety you seek used to be available so have a look on eBay. Also the express card twin slot USB3 adapters do work, I have several in use, I am sure that triple ones are available too.
