I bought an X200 off eBay for my brother who is very happy with it but complains that while the speakers work, headphones don't.
It sounds to me like a physical defect but on the off-chance, has anyone had a similar problem that was due to software / bios (and that they managed to fix)?

firstly I would try different headphones first > reinstall os > replace sub card.

Is his SD card reader working fine? Is the USB port working fine as well near the heaphone jack? Just asking because my built-in SD card reader failed once and I bought the module which looks something like the one on this fleabay ad. I am not affiliated with the seller but could not find a better picture to give you a visual of the module.

Mine cost ~$10 and it was quite easy to replace. Just get the keyboard, palmrest off, unscrew 3-4 screws for the module, disconnect the ribbon connector and replace with the non-defective one. Took me < 10 mins to do.
I cannot say for certain if that would fix the X200 but the modules are not terribly expensive if you are patient and search carefully. Good luck
