I bought a X220 with a missing red Trackpoint cap and I cannot locate a replacement (official or 3rd party) anywhere.
Every one I buy has a 4mm hole but the post in the keyboard is a 3mm square. Am I missing something?

Welcome to the Forum.
There is another piece missing, that goes between the keyboard 'stud' and the trackpoint.

Thank you, I thought there was something missing, my X230 has that piece in yellow.
Does this mean I need a new keyboard or can the missing part be bought separately?

You can try to find it, but I've never seen it for sale (or mentioned) anywhere.
Get the X220 manual from the above HMM link.
Find any old NMB keyboard (from the first column in the HMM parts list) of any language, even faulty ones, that still has the trackpoint/missing part and transfer that part to yours.
Or check with eBayers that sell individual NMB-keys, they might also have that trackpoint-"adapter".
