Password Manager Lost All Passwords

Not sure how or why this happened, but suddenly password manager lost all of my passwords. Anyone ever had this happen to them? Is it possible to retrieve?

Yes, the same thing happened to me last week. I was trying to manually enter a username and password into password manager at the time. The next thing I know all of my passwords were gone!
I was not able to recover them.

I figured it was a Vista related issue because I never had this problem with XP.

This is not right. Im not sure if I want to enter all my passwords again to only have it forget them all. I guess i could back them up, but Id like to find out why this is happening.
