Part Number for Harddisk adapter

What is the part number for the Thinkpad T40 harddisk adapter that is secured to the harddisk by 4 screws and to which the harddisk cover is attached?

Is this adapter required for the Ultraslim bay harddisk adapter?

I haven't found the part number yet, primarily because I haven't had the need to order one.

You don't need one to put a HD in the 2nd HD adapter.

I already checked above but the part number is not listed. Item #5 identifies the harddisk.
