I hope it isn't frustrating that the info is in a link, but I wrote up instructions on my Windows tips blog:

As a preview, the tips involve using a Cardbus UDMA CompactFlash adapter to store some indexes on a UDMA-enabled CF card. Also, uninstalling some background security software helps a lot, but I realize this point is controversial. I got it on Ebay, but it's on Amazon, too.
