Genuine ThinkPad AC Adapter Photos Thread (Will have images)

I'd like to compile photos of genuine ThinkPad AC adapters, for those looking at buying used or lower-cost adapters.

Please only post photos of adapters that you got with a new ThinkPad, directly from IBM/Lenovo, or from an IBM/Lenovo authorized reseller. When you post them, please state the source of the adapter, the part number, and what the adapter is (voltage and wattage.)

Photos of the front, back, top (where the laptop cord exits,) bottom (where the AC cord enters,) and the plug (with any markings on the plug being visible) and securing velcro strap are good photos to have for this.

OK, let's start with mine:

IBM AC power supply, found in the factory T61p box
FRU: 08K8207
Input: 100-220V 50/60Hz
Output: 20V 4.5A (90W)

Please note that the label is wrong (LOL) and I was told that a certain number of ACs were wrongly labelled (how?!?!?)
